People and positions are different things
A position is a role your organization needs doing. A person may or may not be in it. Keeping them apart is what lets you see a vacancy, a role somebody is covering on top of their own, and a role you have written down and never funded.

A lightweight reports-to hierarchy stands in for a full org chart. It is enough to answer who covers what when somebody leaves, which is the question that actually gets asked.

Capacity, honestly
The Organizational Capacity Diagnostic in the Diagnostic Center asks whether you can deliver what you have already committed to. It is the diagnostic most likely to tell an organization something it already knew and had not said out loud.
- Record actual hours, not contracted ones, where the two differ. The gap is the finding.
- Note who is the only person who can do something. Single points of failure in a small nonprofit are usually people, and usually unrecorded.
